When you think of your business do any images come to mind? If so, what are they?
Describe your direct competition; provide addresses to their websites if available.
Describe your target market, gender, age, geography. What are their likes? What are their dislikes? How do you connect with your target audience?
What type of image do you most want to project to your prospective customers/visitors? How would you describe your brand to a friend? (i.e. clean and simple, high tech, fanciful, elegant)
Please provide your motto, mission statement or tag line. Do you want to include that in the logo?
Do you have a specific image you want to include in your logo? Do you prefer your logo to be an icon, text or both

Are there any images or concepts you DO NOT want to see?
Do you have any font preferences? Do you want a serif or non serif font? Serif fonts have small decorative hooks (serifs) at the ends of letter strokes, creating a more traditional look, while sans-serif fonts lack these strokes and have a cleaner, more modern appearance. Common serif examples include Times New Roman and Georgia, while Avenir and Helvetica are examples of sans-serif fonts.
